Smile Line Filler
qd smile line filler
Smile line filler softens the nasolabial folds — the lines running from the sides of the nose down to the corners of the mouth. Hyaluronic acid (HA) can cushion the crease directly, and often the better approach is to support the cheek above it, since a deflated cheek is frequently what deepens the fold. At QD Clinic in Gangnam, Seoul, we look at both the fold and the cheek first, then decide where to place the HA. A session usually takes about 15–30 minutes with little downtime. (Results vary by individual.)

is this for me
Who it’s for
Even if none of these describes you, we will look again at the consultation.
- Nasolabial folds from the nose to the mouth that look deeper with age
- A tired look driven partly by folds and a deflated cheek
- Folds you would like to soften without surgery
- Anyone wanting a natural softening rather than fully erasing the line
Because the smile line moves when you talk and smile, the result is a natural softening rather than a fully flat line, and supporting the cheek is often part of the plan. Suitability is confirmed during consultation.
start with your concern
Before you pick a treatment name, start with the cause of your concern.
Even a treatment with the same name takes on a different role and scope depending on your current structure, the change you want, what you have had before and the recovery time you have.
Where you are now, and where we start
The smile line itself and how it relates to midface support
The change you want, the impression you keep

How it differs from similar options
These all soften the fold or lift the cheek that deepens it, but they work in different ways and suit different goals. The right choice is decided after we assess the fold and cheek.
Health information and recovery schedule
Little downtime; most people return to daily activities the same day. Temporary swelling, redness, tenderness or bruising may occur and usually settles within about 1–2 weeks. Avoid pressing or massaging the area, and skip saunas and intense exercise for a day or two as we advise.
the principle
How it works
Rather than a still photograph, we look first at expression, overall proportion and vascular anatomy, then set the product, the injection layer and the amount conservatively.
HA filler adds volume where it is placed and is gradually absorbed over time. For smile lines, the doctor often supports the cheek first — restoring volume above the fold can lift and reduce the crease at its source — and then places a softer gel directly along the fold where needed, usually with a cannula in a deeper layer to keep it smooth. Treating the cause and the line together tends to look more natural than overfilling the crease alone. Because this is a mobile area, the fold is softened rather than completely erased. HA can be dissolved with hyaluronidase if needed. (Results vary by individual.)

How it works
Water-holding volume
Hyaluronic acid binds water and adds soft volume, used either to support the cheek above the fold or to cushion directly beneath the crease.
Treat the cause, then the line
The cheek above is often supported first, since a deflated cheek deepens the fold; a softer gel is then placed along the crease itself, usually with a blunt cannula in a deeper layer.
Immediate but temporary
Softening shows the same day because the gel fills and cushions the crease; over the following months the body gradually breaks the HA down, so the effect is not permanent.
Adjustable and reversible
Because it is HA, the result can be added to gradually, or dissolved with an enzyme (hyaluronidase) if an adjustment is ever needed.
the qd standard
The same treatment, never the same plan.
Rather than a fashionable name or a fixed package, we weigh the structure found at examination, safety, recovery and the limits within one plan.
- DIAGNOSIS FIRST
Cause before name
We first separate out the tissue and the function behind your concern.
- LESS, PRECISE
Only as much as needed
Rather than an excessive change, we plan the smallest scope with a clear role.
- HONEST LIMIT
The limits, explained too
We explain the change you can expect, the alternatives, individual differences and the risks.

Cleansing, marking and topical numbing for comfort
Small amounts, one stage at a time
Softening appears right away and settles over about 1–2 weeks. Because the smile line moves often, HA here tends to last around 9–12 months, usually less than deep, still areas. A top-up is discussed at consultation. (Results vary by individual.)
What the procedure is like
- Consultation and assessment of the fold and cheek
- Cleansing, marking and topical numbing for comfort
- HA to support the cheek and/or soften the fold, by cannula or needle
- Review of symmetry and aftercare guidance
Time: A session usually takes about 15–30 minutes.

Safety & considerations
Possible reactions
- Temporary swelling, redness or a feeling of fullness
- Tenderness or bruising at the injection points
- Small lumps or slight unevenness, which usually settle or can be smoothed
- Very rarely, filler affecting a blood vessel — this needs prompt medical attention
When to take extra care
- Pregnancy or breastfeeding — let us know at consultation
- Active skin infection, inflammation or a breakout in the treatment area
- A known allergy to hyaluronic acid or lidocaine
- Previous filler, threads or surgery in the area — tell your doctor so placement can be planned
- A bleeding tendency or blood-thinning medication — tell your doctor
Whether treating the fold, the cheek, or both suits you is confirmed during consultation. Effects and their duration vary by individual, and side effects may occur. We explain the warning signs — unusual pain, skin turning pale or dusky, or any change in vision — so you know to contact us promptly.
what to expect
What to expect over time
A general guide to how the days and months after treatment usually unfold. Results vary by individual.
DAY 0
Treatment day
Softening is visible immediately. Mild swelling, redness or tenderness is common, and the area can look fuller than the final result at first.
DAY 1-3
Early settling
Most swelling eases and any bruising begins to fade. Makeup and normal skincare can usually resume as advised.
WEEK 1-2
Settled result
Swelling resolves and the softened fold looks natural at rest and in movement. (Results vary by individual.)
9-12 MONTHS
Duration
Because the smile line moves often, HA here tends to last around 9–12 months, varying with the product and your metabolism. (Results vary by individual.)
TOP-UP
Review & maintenance
Your doctor reviews the result and, if appropriate, discusses timing for a top-up based on your face.

| Smile Line Filler | Collagen-stimulating filler | Thread lift | |
|---|---|---|---|
| Agent | Hyaluronic acid (HA) gel | Calcium hydroxylapatite or poly-L-lactic acid | Absorbable threads (e.g. PDO) |
| What it does | Cushions the fold and supports the cheek above it | Prompts your own collagen to build volume gradually | Repositions and lifts the cheek mechanically |
| Onset | Immediate; settles over about 1–2 weeks | Gradual over weeks to months | Immediate lift; settles over weeks |
| Duration | Around 9–12 months (varies) | Often longer, up to about 2 years (varies) | Months, varying with thread type |
| Reversible? | Yes — dissolvable with hyaluronidase | No specific dissolver | Threads absorb; not easily removed early |
| Downtime | Little; temporary swelling or bruising | Little; similar injection reactions | Some soreness and swelling for a few days |
Neither is “better” — they work differently and suit different goals. The right choice is decided after we assess your skin.
before consultation
Before you decide, the questions people always ask
The groove becomes shallower, but the line itself does not disappear. A smile line sits where the tissue of the cheek has slipped downwards and folded over, so filling the crease alone easily looks unnatural. That is why we first support the flattened part of the cheek and then treat only the groove that still remains.
Because the cause sits higher up while only the result is being filled. If the crease is packed while the cheek is still descended, that spot bulges out and looks awkward when you smile. The line softens naturally only when the volume above it is planned at the same time.
Arteries that supply the face run close to the smile line, so this area calls for care. We check the depth and the position as we go and place small amounts slowly. If you feel severe pain during the treatment, or the skin feels as though it is blanching, please tell us immediately.
This area never stops moving while you talk, eat and smile, so it is absorbed faster than the cheekbones or the temples. How large the difference is depends greatly on the product, the amount placed and how much you speak. If you note how long it took after your first treatment for things to look as they did before, we learn your own absorption rate, and timing the next session becomes easier.
When sagging is clearly visible, the natural order is to lift first and then fill whatever groove is left. The other way round, the lifting can push the filler and change its shape. At the consultation we separate how much comes from sagging and how much from lost volume, and then decide the order with you.
Look at the examination and the plan rather than the treatment name. First, what the clinic checks before deciding — at QD Clinic in Gangnam, Seoul, the first thing we look at for this treatment is “The smile line itself and how it relates to midface support”. Second, whether it explains why it recommends that scope and method, compared with the alternatives. Third, whether it reviews your progress with you when results are expected. Our doctor goes through these three points at consultation before the treatment is decided.
